In Ghana, recent policy reforms and the government\u2019s commitment to Universal Health Coverage have heightened attention on safeguarding patients from avoidable errors and adverse events. Yet, as Augustine Kumah observes, translating this commitment into safer everyday care requires confronting deeply entrenched challenges across infrastructure, workforce, and governance domains.<\/span><\/p><p class=\"p\"><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">Ghana\u2019s mixed delivery model comprises public, private, and faith-based providers coordinated through the Ministry of Health and implemented by the Ghana Health Service. Anchored by a three-tiered network<\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">; <\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">ranging from community-based CHPS compounds to district hospitals and teaching hospitals<\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">, <\/span><b><i><span style=\"font-family: Montserrat;\">the system has achieved notable gains in expanding access. <\/span><\/i><\/b><b><i><span style=\"font-family: Montserrat;\">Nevertheless, uneven resource distribution and capacity gaps between urban and rural facilities continue to undermine care quality.<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u00a0The NHIS, launched in 2003 to eliminate point-of-service payments, has boosted utilization, but persistent inequities in service readiness and quality remain a barrier to safe care delivery.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-b66add4 e-flex e-con-boxed e-con e-parent\" data-id=\"b66add4\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-ff0e9dd elementor-widget elementor-widget-image\" data-id=\"ff0e9dd\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t<figure class=\"wp-caption\">\n\t\t\t\t\t\t\t\t\t\t<img decoding=\"async\" src=\"https:\/\/upload.wikimedia.org\/wikipedia\/commons\/8\/88\/World_Health_Organization_%40_Pregny-Chamb%C3%A9sy_%2850636746028%29.jpg\" title=\"\" alt=\"\" loading=\"lazy\" \/>\t\t\t\t\t\t\t\t\t\t\t<figcaption class=\"widget-image-caption wp-caption-text\">WHO Headquarters, Geneva<\/figcaption>\n\t\t\t\t\t\t\t\t\t\t<\/figure>\n\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-0049094 e-flex e-con-boxed e-con e-parent\" data-id=\"0049094\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-b77e337 elementor-widget elementor-widget-text-editor\" data-id=\"b77e337\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><b><i><span style=\"font-family: Montserrat;\">One of the most urgent patient safety concerns in Ghana is hospital-acquired infection (HAI).<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u00a0A multicenter point-prevalence study conducted in 2016 found that <\/span><b><i><span style=\"font-family: Montserrat;\">8.2 percent of inpatients across ten hospitals suffered HAIs, with surgical site infections accounting for 32.6 percent, bloodstream infections 19.5 percent, and urinary tract infections 18.5 percent of cases.\u00a0<\/span><\/i><\/b><\/p><p><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">By 2024, the economic toll of<\/span><b><i><span style=\"font-family: Montserrat;\">\u00a0these infections was estimated at USD 1.57 billion annually<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u2014nearly 2 percent of Ghana\u2019s GDP\u2014<\/span><b><i><span style=\"font-family: Montserrat;\">while 45 percent of healthcare facilities reported inadequate access to clean water,<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u00a0a basic necessity for effective infection prevention and control (IPC).\u00a0<\/span><span style=\"color: #5d6273; font-family: Montserrat;\">Medication errors and diagnostic failures further compound patient harm. Home-based studies among pediatric caregivers revealed that <\/span><b><i><span style=\"font-family: Montserrat;\">68.4 percent of children experienced at least one medication error, <\/span><\/i><\/b><span style=\"color: #5d6273; font-family: Montserrat;\">principally involving incorrect timing or dosing frequency.\u00a0<\/span><\/p><p><b><i><span style=\"font-family: Montserrat;\">In hospital settings, error rates ranging from 18.2 percent to 27.2 percent have been documented,<\/span><\/i><\/b><span style=\"color: #5d6273; font-family: Montserrat;\">\u00a0with many resulting in adverse outcomes due to gaps in training, standardization, and oversight. <\/span><b><i><span style=\"font-family: Montserrat;\">Misdiagnoses and laboratory mistakes<\/span><\/i><\/b><span style=\"color: #5d6273; font-family: Montserrat;\">\u00a0<\/span><span style=\"color: #5d6273; font-family: Montserrat;\">such as incompatible blood transfusions<\/span><span style=\"color: #5d6273; font-family: Montserrat;\">\u00a0<\/span><b><i><span style=\"font-family: Montserrat;\">underscore the need for systematic mortality audits and root-cause analyses <\/span><\/i><\/b><span style=\"color: #5d6273; font-family: Montserrat;\">to prevent recurrence.<\/span><\/p><p><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">Beyond clinical lapses, systemic constraints pose formidable obstacles to safer care. <\/span><b><i><span style=\"font-family: Montserrat;\">Ghana\u2019s doctor-to-patient ratio remains an estimated 1:8,000<\/span><\/i><\/b><b><i><span style=\"font-family: Montserrat;\">, <\/span><\/i><\/b><b><i><span style=\"font-family: Montserrat;\">well below the WHO\u2019s recommended 1:1,00<\/span><\/i><\/b><b><i><span style=\"font-family: Montserrat;\">0,<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u00a0<\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">while overburdened nurses report high burnout and fatigue levels that elevate the risk of errors.\u00a0<\/span><\/p><p><b><i><span style=\"font-family: Montserrat;\">Only 5 percent of primary-level facilities possess complete basic examination tools, <\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">hampering early detection of critical conditions. <\/span><b><i><span style=\"font-family: Montserrat;\">Financially, health received just 6.9 percent of the 2024 national budget<\/span><\/i><\/b><b><i><span style=\"font-family: Montserrat;\">;<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">\u00a0<\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">far short of the Abuja Declaration target<\/span><span style=\"mso-spacerun: 'yes'; font-family: Montserrat; mso-fareast-font-family: SimSun; font-size: 10,0000pt; mso-font-kerning: 0,0000pt;\">, <\/span><b><i><span style=\"font-family: Montserrat;\">with 71 percent of that allocation consumed by salaries,<\/span><\/i><\/b><span style=\"mso-spacerun: 'yes'; font-family: Montserrat;
